Support Coordinator – Stroke Recovery Service (North Lincolnshire)

About the role

This part‑time position supports stroke survivors and their families across North Lincolnshire, helping them navigate the recovery pathway. You will work from home while travelling to homes and community settings to deliver personalised assistance.

Key responsibilities

• Make introductory calls to stroke survivors and carers to identify needs and map support pathways.
• Maintain confidential, accurate records on the CRM database in line with case‑management principles.
• Organise and deliver community engagement activities, including awareness events and service presentations.
• Provide personalised information, advice and support through home visits and other contact methods using a person‑centred approach.
• Collaborate with health professionals and partner organisations to ensure coordinated care throughout the stroke pathway.

Required profile

• Experience delivering person‑centred support to individuals or communities.
• Proven ability to improve outcomes for service users.
• Comfort using technology and IT systems to keep timely, accurate records.
• Experience working collaboratively with professionals across varied settings.
• Ability to deliver presentations and organise local events.
• Willingness to travel extensively across a large geographical area.

• Salary circa £17,000 per annum (part‑time) – full‑time equivalent £28,300.
